All students wanting to participate in a sport must have an updated physical within the last year. For example: a physical done on 11/20/2022 is good until 11/30/2023. If your physical was done on 11/20/2023 and the sport starts on 11/24/2023, you are cleared for that season. However, you will need an updated physical for the next season you wish to participate in.
There is a new program that we are using to clear students who want to participate in sports. It’s called Aktivate. You must create an account and register your child on this site. Before each season, you must sign them up for the current sport they wish to play by filling out all tabs. There are three different sections after you register your child that MUST be filled out completely. The fourth section will ask you to upload your child’s physical. You can do this but do not have to. If you upload the physical to the Aktivate site, a PAPER COPY is still required to be on file in the nurse’s office. The physical form that is required to be filled out is called REQUIRED NYS SCHOOL HEALTH EXAMINATION FORM. All doctor’s offices have this and know that it needs to be filled out.
The Health History form that needs to be filled out at the beginning of each sport is part of the forms that must be completed on this site. It can be completed by a parent no earlier than 30 days before the start of the season.
Physicals are NOT offered at the school anymore. Therefore, your own Primary Care Provider will need to do your physical. If you do not have a PCP and need more information on where to get a physical completed, please email me.
You can email, fax, mail, or drop off your physical forms in the High School Main Office.
