All students wanting to participate in a sport must have an updated physical within the last year. Physicals are good for one year and expire at the end of the month that they were received on. For example, a physical that was done on 11/20/2023 is good until 11/30/2024. If your child's physical was done on 11/20/2023 and the sport starts on 11/24/2023, your child is cleared for that season. However, they will need an updated physical for the next season they wish to participate in.
There is a new program that we are using to clear students who want to participate in sports. It’s called Aktivate. You must create an account and register your child on this site. There are three different sections after you register your child that MUST be filled out completely. The fourth section will ask you to upload your child’s physical. You can do this but do not have to. If you do not upload it to the website, a paper copy needs to be sent to the nurses' office to have on file. The physical form that is required to be filled out is called REQUIRED NYS SCHOOL HEALTH EXAMINATION FORM. All doctor’s offices have this and know that it needs to be filled out. Before each season, do not register a second time, just simply sign your child up for the new sport they wish to play. One of the tabs that needs to be filled out is a Health History form. This cannot be completed any earlier than 30 days before the start of the season.
Physicals are NOT offered at the school anymore. Therefore, your own Primary Care Physician will need to do your physical. If you do not have a PCP and need more information on where to get a physical completed, please email me. You can email, fax, mail, or drop off your physical forms in the High School Main Office.
